I confirm that I am at least 18 years of age.*

I understand that SportsKlub is built on fun, inclusion, friendship, and shared experiences, and that events are intended to be welcoming, light-hearted, and enjoyable for everyone, regardless of skill level or background. By registering, I agree to play respectfully and within the spirit of the game, treating teammates, opponents, officials, volunteers, and staff with courtesy and sportsmanship at all times. *

I understand that failure to conduct myself respectfully and within the spirit of the game may result in removal from an event without refund.*

I understand that participation in SportsKlub events and activities involves inherent risks, including but not limited to physical injury, illness, permanent disability, or death. These risks may arise from my own actions or inactions, the actions or inactions of others, the condition of facilities or equipment, weather conditions, or other factors known or unknown.*

I voluntarily choose to participate and freely assume all risks, both known and unknown, associated with my participation in SportsKlub activities.*

In consideration of being permitted to participate in SportsKlub events, I hereby waive, release, and discharge SportsKlub, including its owners, organizers, volunteers, staff, officials, sponsors, partners, and venue providers, from any and all claims, demands, causes of action, damages, or liabilities arising out of or related to my participation, whether caused by negligence or otherwise, to the fullest extent permitted by law.*

I confirm that I am physically and medically able to participate in SportsKlub activities. I accept full responsibility for any medical expenses incurred as a result of injury, illness, or emergency treatment arising from participation. I understand that SportsKlub does not provide medical insurance coverage for participants.*

I agree to indemnify and hold harmless SportsKlub and its ownership from any claims, liabilities, damages, or costs (including legal fees) arising from my participation, including any claims brought by third parties.*

I acknowledge and agree that SportsKlub events may be photographed or recorded for promotional marketing, social media, website, and archival purposes. I hereby grant SportsKlub the unrestricted right to capture, use, reproduce, publish, and distribute photographs, video recordings, and audio recordings of me taken during participation of SportsKlub activities.*

I have read and fully understand this Waiver and Release of Liability. I understand that by signing this document, I am giving up certain legal rights, including the right to sue. I sign this agreement voluntarily and without coercion.*
